TB Screening Requirements

All incoming students must complete a tuberculosis (TB) screening questionnaire. Results of this evaluation, in addition to documentation of required vaccines (or evidence of immunity to these diseases) must be submitted to the University Health Center through the myUOHealth student portal. Incoming students who are assessed as potentially at higher risk for TB infection, based on their responses to the screening questionnaire, will be required to undergo further evaluation and testing. The University of Oregon requires that higher-risk students receive TB testing with either a blood test (Quantiferon or T-Spot) or tuberculin skin test (TST; PPD).

If you are identified as high risk, you will be notified with further instructions. TB testing performed outside the health center must be completed no earlier than six months prior to arrival on campus. Otherwise a Quantiferon blood test can be done at the health center upon arrival.

Submitting Documentation

Complete your TB screening questionnaire online through the myUOHealth student portal. To log in to myUOHealth:

  • User ID is your Duck ID—the University of Oregon e-mail address.
  • Password is your e-mail password.
  • Complete the TB screening questionnaire under the "Forms" tab.

If you are unable to access the student portal, you may submit your TB screening questionnaire by mail, e-mail, or fax.

Any testing performed internationally requires results translated into English before submission.

A hold will be placed on the registration process of students who are non-compliant with the MMR immunization requirements or completing the TB screening questionnaire. For international students, the hold will be placed on registration for their first term at the University of Oregon. For U.S. citizens, the hold will be placed on registration for their second term.
