Confidentiality is central to the effective provisions of healthcare services. A confidential medical record is established and maintained for every patient who receives care at University Health Services. To obtain a copy of your medical records, you will need to complete a release of information form. You can either complete it online through the myUOHealth student portal under the "Forms" tab, or you can download the form, complete it, and submit it through myUOHealth or bring a paper copy to the front desk of health services.
Assistance with this form is available Monday through Friday from 9:00 a.m. to 5:00 p.m. Please allow 10 business days for the processing of your request.
